This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by May 25, 2024, or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n

3.13(b)(3) (Non-Critical) – Veterinary care for dogs.
The facility does not have the dates of administration for treatments recorded in the medical records. A 6-week-old female dachshund puppy (microchip #0020) is currently being treated for a left eye injury. The facility representative stated the issue was observed on 3-July-2026 and per the program of veterinary care has been applying neopolybac ointment three times daily. When inquiring where this is being documented on a medical record the representative stated she has not documented the administration of the medication for this puppy. Failure to record the dates for all treatments administered does not allow for APHIS officials to determine if adequate veterinary care has been provided for animals covered under the Animal Welfare Act. The licensee must ensure that the names of all vaccines and treatments administered, and the dates of administration are recorded and maintained. Correct by from this day 21-July-2026 and going forward. This inspection and exit interview were conducted with the facility representative. n
