A responsible adult was not available to accompany APHIS Officials during the inspection process at 1:05 pm on March 03, 2026. The inspector attempted to contact the licensee by phone, text, and rang the front doorbell. Failure to provide access to the facility, animals, and records for inspection is a violation of the AWA and regulations. Continued failure to provide access is serious and may result in enforcement action. End Section

2.50(a)(2) (Non-Critical) – Time and method of identification.
Puppies on premises at the time of inspection did not have official tags to identify them. Five of the six puppies did not have a microchip or official tattoo nor were wearing an official tag or plastic collar containing the required information. All dogs including puppies need to be identified in order to allow for accurate identification for legal and medical purposes. The facility shall identify all live dogs and puppies on the premises using an official method of identification as described in the regulations. The five puppies without official tags need to be identified using a microchip, official tattoo, official tag or plastic collar with the required official identification pursuant to Sec. 2.51. Correct by: 08/15/2026.
2.75(a)(2) (Non-Critical) – Records: Dealers and exhibitors.
The facility does not maintain complete Record of Acquisition of Dogs and Cats on hand form (APHIS form 7005) or a Record of Disposition of Dogs and Cats form (APHIS form 7006) for the dogs and puppies born and sold at the facility. Eleven puppies have been born at the facility since the last inspection and only two are documented on the APHIS 7005 form. There is no APHIS 7006 forms created or maintained for the three adults and five puppies sold since the last inspection. The acquisition and disposition records are instead being kept on documents they have created. They do not have a variance in place that allows them to use other forms. Failure to use APHIS form 7005 and 7006 does not allow for proper traceability and management of regulated animals at a facility. Each dealer and exhibitor shall use the Record of Acquisition of Dogs and Cats on Hand (APHIS Form 7005) and Record of Disposition of Dogs and Cats (APHIS Form 7006) to make, keep, and maintain the information required by paragraph (a)(1) of this section. Provided, that if a dealer or exhibitor who uses a computerized recordkeeping system believes that APHIS Form 7005 and APHIS Form 7006 are unsuitable for him or her to make, keep, and maintain the information required by paragraph (a)(1) of this section, the dealer or exhibitor may request a variance from the requirement to use APHIS Form 7005 and APHIS form 7006. Correct by ensuring all required information is fully and correctly documented on an APHIS Form 7005 and 7006, or by obtaining a variance. Correct by: 08/15/2026.
3.13(b) (Non-Critical) – Veterinary care for dogs.
The licensee did not maintain complete medical records for dogs. The APHIS official was provided a record of vaccines, given by the licensee, for both dogs and puppies present during the inspection, and a list of reminder dates for the dogs’ exams, rabies vaccines, heartworm tests, and fecal tests provided from the veterinarian’s office. No findings from the screening tests (internal parasite and heartworm tests) were listed. Also, Mocha, an adult female, was diagnosed with cherry eye by the attending veterinarian, per the licensee, but they did not have records documenting the date and description of the problem, examination findings, or the plan for her treatment and care. Keeping up to date medical records is required to verify routine preventative care, annual exam findings and appropriate treatment of any medical conditions/injuries for the dogs. The facility must keep copies of medical records for dogs that include the following required information: identifying information about each dog, all preventative and routine husbandry procedures performed, information about problems identified, treatments/vaccinations administered, and results of all tests and examinations performed as well as make the records available for APHIS inspection. Correct by 08/15/2026. This inspection and exit interview were conducted with the licensee. n
