The majority of the APHIS 7006 forms did not list the method of transportation of the carrier, intermediate handler, or name of the person who transported the dog or puppy. Failure to record the method of transportation makes it difficult to know if the buyer, seller, or commercial shipper transported the puppy or puppies. Correct by recording the method of transportation on APHIS form 7006, including the name of the carrier, intermediate handler, or owner’s name if a privately owned vehicle was used to transport the dog or puppy. Correct by October 5, 2023. This inspection and exit interview were conducted with the licensee. n
