2.75(a)(1)(iv) (Non-Critical) – Records: Dealers and exhibitors.
The record of sale for 3 puppies (APHIS 7006) did not include the correct name and address of the person that the puppies were sold or given. Complete and accurate disposition records are required for proper management and traceability of the regulated animals. Each dealer shall make, keep, and maintain records or forms which fully and correctly disclose the information in 2.75(a)(1)(i) through 2.75(a)(1)(ix) for each dog purchased or otherwise acquired, owned, held, or otherwise in his or her possession or under his or her control, or which is transported, euthanized, sold, or otherwise disposed of by that dealer. The applicant must ensure accurate records are created and maintained for all dogs at the facility. Correct by 30 October 2024 forward. This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspection(s) or by 29 December 2024 or the applicant will forfeit the license fee and must wait 6 months to reapply.
