3.13(a)(2) (Non-Critical) – Veterinary care for dogs.
A complete physical examination of each dog at the facility has not been completed by the Attending Veterinarian (AV) within the last 12 months. The AV visited the facility in June and November 2024, however, a head to tail physical exam has not been completed for each of the dogs since January 2024. This exam is important to ensure that all dogs at the facility are in good health and that medical conditions are addressed in a timely manner. A complete physical examination from head to tail of each dog by the Attending Veterinarian must be performed not less than once every 12 months. This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by May 4, 2025 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires (April 6, 2025). n
