Two enclosures had small areas of loose or broken wire strands that the dogs could contact. Loose or broken wire strands could injure animals. Ensure that enclosures are maintained free from potentially sharp points or edges. Note: The facility added these items to the maintenance notes and a plan is in place to repair these areas. Ensure these areas are repaired/replaced by 9/29/2022. This inspection and exit interview were conducted with a facility representative. n

2.134(b) (Non-Critical) – Contingency planning.
The licensee didn’t document an annual review of the facility contingency plan since 2/12/2024. The licensee reports reviewing it around May 2026 and found no updates were needed so it was maintained the same. Documenting the annual review of the contingency plan is required and helps ensure a facility is ready for adverse events. Ensure to maintain documentation of contingency plan annual reviews, including documenting any amendments or changes made to the plan since the previous year’s review, such as changes made as a result of recently predicted, but historically unforeseen, circumstances (e.g., weather extremes). ***Correct by 7/31/2026.
