3.13(a)(4) (Non-Critical) – Veterinary care for dogs.
The dogs were all examined for their annual physical exam in October 2021 and several dogs are listed as having moderate to severe tarter with missing teeth. No preventative care or treatment plan to address the dental disease has been implemented. The licensee needs to contact the vet for further guidance in how to proceed with preventative care and treatment for these animals to ensure their dental issues are addressed. Four dogs were identified to have heavily matted fur, around the feet, underneath the neck and along the belly or back area. Matted fur pulls and separates from the skin causing skin irritations and is no longer an effective insulator against the elements. Additionally, these dogs have long nails that need to be trimmed. These dogs are: Bichon id #027 (Bun), Bichon #022 (Biscuit), Poodle # 014, and Bichon #007 (Frank). These animals all need to be groomed to ensure they have healthy un-matted hair coats for their health and comfort. Part of preventative care for this section is to not only identify concerns of animal health but develop a plan that treats identified issues with hair coats, nails, and teeth. 2.134(a) (Non-Critical) – Contingency planning.
There was no written contingency plan on file at the facility. Failure to have a written contingency plan can result in lack of readiness for emergencies or disasters. Facilities must develop, document, and follow an appropriate plan to provide for the humane handling, treatment, transportation, housing, and care of their animals in the event of an emergency or disaster. Correct by: April 4, 2023

3.13(b)(3) (Non-Critical) – Veterinary care for dogs.
The licensee is keeping records of rabies vaccinations but not of other yearly vaccinations (7 way vaccine with distemper/parvo). Failure to document all vaccines can result in not keeping dogs in the kennel current on vaccinations. Dealers must keep copies of medical records for dogs that include the names and dates of all vaccines administered. 3.11(a) (Non-Critical) – Cleaning, sanitization, housekeeping, and pest control.
The inside runs with shavings for females and puppies are not being spot cleaned daily causing a build up of feces and urine soaked shavings on the floor. The whelping box housing a pug had several days’ worth of feces in the box that was not removed. These enclosures must be spot cleaned daily to keep the animals from becoming soiled and for their comfort. Correct by: Feb 16, 2024

3.11(c) (Non-Critical) – Cleaning, sanitization, housekeeping, and pest control.
In general, there is a lot of dust, debris and clutter around the kennel that needs to be cleaned. Areas between the whelping boxes and the wall had excess dust and cobwebs that need to be removed. There were multiple items (used syringes, bottles etc.) stored on top of a whelping box that were covered in dust and there were miscellaneous items stored around the food supplies and spilled food on the floor that was not removed. All this clutter, dust and debris need to be addressed to keep the kennel clean to promote good housekeeping practices. 3.10(a) (Direct) – Watering
During the inspection, puppy 2444 was observed slumping against an empty bowl placed under the enclosure’s drinking spout. It appeared that it could not use the spout given the size of the puppy compared to its larger littermates. The facility representative was asked to fill the bowl with water, after which the puppy proceeded to drink for 3 minutes. On examination, the puppy exhibited increased skin tenting consistent with dehydration. Inability to access water can rapidly cause dehydration in puppies. The facility must ensure continuous access to water for all dogs. Correct by 6/27/24. A complete routine inspection was conducted starting on 6/27/24. This inspection report is limited to the Direct non- compliances identified during that inspection. An additional inspection report will be delivered with the other issues identified during this inspection. This inspection was conducted with the licensee. The exit briefing was conducted on 6/27/24 with the licensee. n

2.40(a)(2) (Non-Critical) – Attending veterinarian and adequate veterinary care (dealers and exhibitors).
While the facility provides deworming and ectoparasite prevention for dogs on the premises, it does not provide heartworm prevention as directed by the written program of veterinary care. Deviation from the program of veterinary care may increase risk of heartworm exposure and disease. The facility must assure the veterinarian has appropriate authority to ensure adequate care by adhering to the plan’s requirements. Correct by 7/18/24.

2.40(b)(2) (Non-Critical) – Attending veterinarian and adequate veterinary care (dealers and exhibitors).
The facility has administered multiple medications and supplements that were out of date, including a deworming medication with an expiration date of May, 2024 and a probiotic with an expiration date of June, 2023. The use of expired medication is not considered standard of care and could lose potency and be less effective. This could cause animals unnecessary harm or distress. The facility must correct this issue. Correct by 7/5/24.

3.8 (Non-Critical) – Exercise for dogs.
The facility does not currently maintain a written plan to provide dogs with the opportunity for exercise. While the dogs do have access to sufficiently sized runs, maintaining written procedures helps ensure these needs are met for all dogs on the premises. The facility must develop an exercise plan approved by the attending veterinarian. Correct by July 25, 2024. This is a continuation of the report for the routine inspection conducted 6/27/24 and contains the remaining non- compliances found during the inspection. The exit briefing for the items included on this report was conducted on 6/27/24. This inspection was conducted with the licensee on 6/27/24. The exit interview was conducted with the licensee on 6/27/24. n

While the facility seeks care related to new issues identified pursuant to its written program of veterinary care, the facility does not maintain records of these problems, examination findings, and subsequent treatments. This could result in deviation from the prescribed treatments and affect the standard of care given. The facility must record dates and descriptions of problems, findings, test results, and treatments and procedures. Correct by 7/17/24. 2.40(b)(3) (Non-Critical) – Attending veterinarian and adequate veterinary care (dealers and exhibitors).
While the facility identified and initiated treatment for 2 dogs (identified as 024 and 038) found to have skin injuries after a fight between the 2, these problems were not communicated to the Attending Veterinarian. Further, the written program of veterinary care does not provide a protocol for managing or monitoring injuries prior to veterinary consultation. Failure to notify the Attending Veterinarian regarding health problems, or failure to follow the written program of veterinary care, may affect the health and well-being of the animals. Facilities must maintain a mechanism of direct and frequent communication to convey accurate information regarding animal problems in a timely manner. Correct by September 26, 2024.

3.13(b)(2) (Non-Critical [REPEAT]) – Veterinary care for dogs.
The facility does not currently maintain records documenting the dates and descriptions of new problems identified, including: Two dogs (038 and 024) with skin injuries sustained in a fight approximately 1 week prior to inspection. The dogs were immediately separated and the resultant scabbing and hair loss to each dog’s face, and the forearm of 038, have since been treated with topical petroleum jelly. The facility also does not maintain records of when treatment was applied. One dog (003) found unable to move its hind legs. While a veterinarian was consulted and visited the facility to treat the dog, the facility does not maintain a description or record of when the problem was identified or communicated to a veterinarian. Documentation of new problems and treatments helps assure these problems are being promptly identified and treated appropriately. Facilities must maintain records of dates and descriptions of new problems, examination findings, test results, treatment plans, and treatments administered or performed. 2.40(b)(3) (Non-Critical) – Attending veterinarian and adequate veterinary care (dealers and exhibitors).
On April 5th, 2025, a female adult cavalier dog, identified #021, was found deceased, but this finding was not reported to the Attending Veterinarian. As such, a necropsy or other follow-up guidance was not provided. The facility reports there were no overt signs of illness or injury prior to death or upon discovery, nor is there any such indication in facility records; no other animals were affected. Failure to provide timely information regarding animal health and welfare to the Attending Veterinarian may compromise oversight of veterinary care. As such, the facility must assure the Attending Veterinarian is alerted to this issue and must continue to provide for frequent and direct communication of welfare information to the Attending Veterinarian. This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by June 9, 2025 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n

3.11(c) (Non-Critical) – Cleaning, sanitization, housekeeping, and pest control.
Housekeeping at the facility needs to be addressed. There are old feces left in empty enclosures that have not been cleaned and removed. There is excess feces and shavings underneath a section of whelping boxes not in use with a dead mouse on the floor and rodent droppings throughout the area. Excess feces left and not cleaned in the area can contribute to a smell in the kennel. These areas of the kennel must be kept clean and in good repair to facilitate good husbandry practices. Correct by: March 15, 2026

3.11(d) (Non-Critical) – Cleaning, sanitization, housekeeping, and pest control.
The facility does not have an effective method of pest management in the kennel. There was evidence of mice via droppings all around the food storage area and along the walls in the kennel by the whelping boxes and in the empty dirty enclosures. Pest management must be addressed to reduce and control the number of rodents in the area to protect the food supplies from potential contamination and to reduce risk to the dogs. Correct by: March 18, 2026

3.13(a)(2) (Non-Critical) – Veterinary care for dogs.
The last complete physical exam done on the dogs from head to tail was performed on Jan 15, 2025. This exam is to be done by the attending veterinarian not less than once every 12 months. The licensee needs to contact the vet and schedule this exam to meet the requirements of the regulation and to assure that the dogs are examined to note any changes in health that may need to be addressed. Correct by: April 13, 2026 This inspection and exit interview were conducted with the licensee. n
