Exposed floor drains which were lacking a cover were observed in an enclosure housing four dogs. The holes were large enough to allow the dogs’ feet to fall through, which could potentially cause injury. Enclosures need to be constructed and maintained in a manner which will protect the dogs from injury. 2.75 (Non-Critical) – Records: Dealers and exhibitors.
The Record of Acquisition and Dogs and on Hand (APHIS form 7005) had not been update to include a newly acquired 10 week old puppy. The licensee had acquired a puppy two weeks ago in order to expand the breeding stock, however none of the required information had been documented. Records must be maintained for all dogs in the kennel and must be updated with the required information to accurately reflect the current inventory. The licensee must correct this issue by updating and maintaining a current APHIS 7005 form. ********Correct by April 1, 2025

2.134(b) (Non-Critical) – Contingency planning.
A Contingency Plan had been documented for the facility, however annual reviews are not being conducted as required. The facility must review the plan on an annual basis, and the date of review and plan revisions, if any, must be documented.
