3.13(a)(3) (Non-Critical) – Veterinary care for dogs.
The facility is not following the written Program of Veterinary Care (PVC). The PVC states to administer a rabies vaccine at 16 weeks of age. Adult male Shih Tzu (#0395), born 9/3/2025, never received this vaccination. Failure to administer required vaccines in accordance with a veterinary approved schedule may result in animals to not be adequately protected against disease and/or can cause pain and distress. Correct by ensuring all animals are vaccinated in accordance with the schedule approved by the Attending Veterinarian. Correct by 14-APRIL-2026.
3.13(b)(3) (Non-Critical) – Veterinary care for dogs.
The facility does not maintain complete medical records for all dogs. The facility states that the following animals were up to date on Parvovirus and Distemper vaccinations but could not provide documentation: Adult female Yorkshire Bichon Mix (#2125), Adult female Lhasa Apso (#3198), Adult female Yorkshire Bichon Mix (#3417), Adult female Shih Tzu (#6577), Adult female Lhasa Apso (#9870), and Adult female Lhasa Apso (#0355). Failure to maintain adequate medical records may interfere with the ability of APHIS officials to determine if the animals are receiving adequate veterinary care. Complete medical records must be maintained and include the names of all vaccines and treatments administered and the dates of administration. Correct by documenting all treatments given and ensure that medical records are complete and contain the required information. Correct by 14-APRIL-2026.
