3.13(a)(3) (Non-Critical) – Veterinary care for dogs.
The facility has not performed rabies vaccinations on four adult dogs in the kennel in accordance with the schedule approved by the Attending Veterinarian. The dogs that were not vaccinated for rabies are Ellie (microchip number 49599), Star (microchip number 67567), Carly (microchip number 58317), and Petunia (microchip number 49500). Failure to follow a written and Attending Veterinarian approved schedule for sampling and treatment of parasites and vaccinations for contagious and deadly diseases can result in a delay of treatment and disease exposure to the dogs. Correct by ensuring that vaccinations for contagious and/or deadly diseases and sampling and treatment of parasites and other pests in accordance with a schedule approved by the Attending Veterinarian is followed. This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by 15-July-2025 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n

3.13(a)(3) (Non-Critical) – Veterinary care for dogs.
The facility is not following the program of veterinary care for vaccinations. The Program of Veterinary Care currently prescribes dogs will be vaccinated for Distemper/Hepatitis/Parvovirus/Leptospirosis vaccines at 9 weeks of age, 12 weeks of age, 15 weeks of age, and annually going forward. The available medical record for a 14-week-old female Standard Poodle puppy named Charlotte (microchip # 6016) born on 30-March-2026, does not show a vaccine containing leptospirosis has been administered. The last vaccine administered per available medical records was a 5-way vaccine given at 6 weeks of age on 14-May-2026. Failure to follow the preventative care schedule for vaccinations in the Program of Veterinary Care does not ensure the dogs are receiving appropriate protection against contagious and deadly diseases such as distemper, parvo, leptospirosis, and rabies which can lead to illness and discomfort to the dogs. The facility must follow the vaccination schedule approved by the attending veterinarian in the Program of Veterinary Care to protect the dogs from deadly or contagious diseases. Correct by 16-July-2026. This inspection and exit interview were conducted with the facility representative. n
