* The record of dogs on hand was incomplete. The record did not have completed names and addresses from where the animals were acquired for 12 of the 13 dogs present. The licensee shall make, keep, and maintain records or forms which fully and correctly disclose the required information required by section 2.75 of this subpart concerning each dog or cat purchased or otherwise acquired, owned, held in his or her possession or under his or her control, or which is transported, euthanized, sold, or otherwise disposed of by that dealer. * To be corrected by: 9/9/22 This inspection and exit interview were conducted with licensee. n

2.40(a)(1) (Non-Critical [REPEAT]) – Attending veterinarian and adequate veterinary care (dealers and exhibitors).
* Upon Request the Program of Veterinary Care(PVC) with the new Attending Veterinarian(AV) was incomplete. An incomplete PVC can affect the health of the dogs. For appropriate veterinary care and promote the health and well-being of the 18 adult dogs and 25 puppies, an complete PVC must be developed by the part time AV. This PVC must be available to APHIS officials upon request.

2.134(a) (Non-Critical) – Contingency planning.
* An contingency plan was not made available to APHIS officials upon request. In the event of an disaster, this could be detrimental to the good health and well-being of the dogs. For the health and well-being of the 18 adult dogs and 25 puppies, an contingency plan must be developed and address and or identify situations that can be trigger by an event. This plan must be available to APHIS officials upon request. This is a (1st) Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within (2 ) more inspection(s) or by (9-December-2023) or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n
