3.6(a)(2) (Non-Critical) – Primary enclosures.
1. A hole was observed in the wooden floor in an enclosure housing 2 dogs. This could lead to injury since the hole was large enough to allow the dogs’ legs to fall through. Enclosures must be kept in good repair to avoid injury to the animals. ****Correct by April 1, 2021 2. A piece of wood serving as a support beam in one enclosure was observed with worn jagged edges. In addition, the tips of 2 screws were exposed and facing into the enclosure. This condition could injure the dogs. The enclosure needs to be kept in good repair in order to protect the dogs from injury. The screws were removed and this was corrected at the time of inspection. *******Corrected at time of inspection

3.11(b)(2) (Non-Critical) – Cleaning, sanitization, housekeeping, and pest control.
Some food bowls were observed with an accumulation of debris. Feeding out of unsanitary bowls could cause the dogs to become ill and affect their health. 2.50(b) (Non-Critical) – Time and method of identification.
Several adult French Bulldogs and their puppies, as well as several enclosures of weaned puppies did not have proper methods of identification. Cage cards are utilized for inventory/birth records and ID purposes, and the name of the adult female dogs and the date of birth of the puppies was recorded, however most of the required information was not included. When cage cards are used for identification they must contain the required information such as dams ID, the USDA certificate number, a unique number identifying each puppy, a description of each puppy including sex and color, and the date of disposition. The licensee must document and display this identification information as required by this section of the AWA. *******Correct by 3/1/25

2.75(a)(2) (Non-Critical) – Records: Dealers and exhibitors.
At the time of inspection, documentation was reviewed for two recently acquired German Shepherd dogs, however the Record of Acquisition of Dogs and Cats on Hand (APHIS Form 7005) had not been updated. In addition, several adult French bulldogs had not been added to the APHIS 7005 Form. Record must be updated to show all dog acquisitions and must be kept current to accurately reflect the inventory in the kennel. The licensee must correct this issue and update the APHIS 7005 document. *****Correct by 3/1/25 This inspection was a site approval inspection. This site is not ready for approval to conduct activity covered under your current Animal Welfare Act License. Conducting regulated activities at a non-approved site is a violation of the Animal Welfare Act. Please contact your inspector to schedule your next new site approval inspection. This inspection and exit interview were conducted with the licensee. n

The licensee disposed of the expired medications at the time of the inspection. This is a 1st Re-License Inspection for a Class “B” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by July 28, 2025-60 days counted from 1st RL Inspection or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n
