The medical records for 154 dogs are incomplete. The physical exams for these dogs completed within the last 12 months are not accounted for in the medical records. The Attending Veterinarian verified that all physical exams were completed but not documented in the medical records for the dogs. Complete medical records allow the attending veterinarian, the licensee and all animal caretakers to ensure that the medical needs of the dogs are accounted for and addressed. The licensee must keep copies of medical records for dogs and make the records available for APHIS inspection. These records must include the dates and findings/results of all screening, routine, or other required or recommended tests or examinations. To be corrected by 8 April 2024.
