3.13(b)(3) (Non-Critical) – Veterinary care for dogs.
The facility does not have the names of treatments, nor the dates of administration recorded in the medical records. While reviewing medical records for vaccines there were 5 adult dogs found to not have a current medical record for the administration of the rabies vaccine. The facility representative states the vaccines were administered, and these dogs are up to date, but he forgot to document this on a medical record. Additionally, the herd record for the annual DAPP/Lepto (7-way) vaccine has 16 adult dogs that the record states were administered the vaccine when their puppies were weaned but does not include the date(s) of administration. Failure to record the names and dates for all treatments administered does not allow for APHIS officials to determine if adequate veterinary care has been provided for animals covered under the Animal Welfare Act. The licensee must ensure that the names of all vaccines and treatments administered, and the dates of administration are recorded and maintained. Correct from this day 31 December 2025 and going forward. This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspection(s) or by 1 March 2025 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n
