3.6(a)(2)(ii) (Non-Critical) – Primary enclosures.
A female pomeranian/chihuahua mix was seen jumping over and out of the enclosure during inspection. The facility reports that this dog doesn’t wander from the property but has learned to jump out at times. A dog that is not safely contained could get injured. Ensure that the enclosures securely contain each dog. Correct by 1/14/2022.

3.13(a) (Non-Critical) – Veterinary care for dogs.
The written Program of Veterinary Care does not describe a plan for physical exams of each dog and the vaccinations for adult dogs for parvo and distemper were due Nov 28, 2021 per records and the Program of Veterinary Care (PVC) but have not been completed. For the health of the dogs; a written plan for physical exams of all dogs is now required and adult vaccines are required to be completed on time. Ensure that the written PVC is updated with required information and that adult vaccines are completed as described in the PVC. Correct by 1/21/2022 This inspection and exit interview were conducted with a facility representative. End Section

3.8 (Non-Critical) – Exercise for dogs.
The facility has an exercise program and adequate space for dogs; however the previously approved plan has not been approved by the new Attending Veterinarian (AV). An exercise plan approved by the AV helps ensure that dogs are provided with adequate space and exercise opportunities. Ensure that the written Exercise Plan for dogs is approved by the AV. ***Correct by 4/11/2024.

3.13(a)(3) (Non-Critical) – Veterinary care for dogs.
The facility has not had the attending veterinarian (AV) take adult dog fecal samples (for parasite screening) as required. Sampling for parasites with fecal samples at the schedule indicated by the AV helps determine if the current de-worming program is working. Ensure to work with the attending veterinarian at getting dogs sampled and keep test results as required. ***Correct by 4/11/2024. This inspection and exit interview were conducted with the licensee. n

2.54 (Non-Critical) – Lost tags.
Three dogs were observed to not have their identification numbers; the licensee reports some dogs have lost their tags recently. Identification numbers are required and help track husbandry and health of dogs. If a dog loses a tag, try to find it or affix a new official tag as required in the regulations. ***Correct by 4/25/2025

2.134(a) (Non-Critical) – Contingency planning.
A written contingency plan/program was not available for inspection. Maintaining a documented contingency plan for emergencies and natural disasters is required and helps a facility prepare for an emergency if it happens. Ensure to document and maintain a written contingency plan to provide for the humane handling, treatment, transportation, housing, and care of their animals in the event of an emergency or disaster. ***Correct by May 9, 2025. This inspection and exit interview were conducted with a facility representative. n
