The facility has not followed vaccination protocol outlined in the program of veterinary care. The facility has not administered the rabies vaccine to a female miniature schnauzer microchip number 29715 and a female cavalier king charles spaniel microchip number 29704 in accordance with the approved vaccination schedule. Failure to provide vaccinations on a schedule approved by the attending veterinarian leads to an increase risk of contagious and/or deadly diseases in the dogs. Correct by ensuring that vaccinations for contagious and/or deadly diseases in accordance with a schedule approved by the attending veterinarian are implemented. This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by 19-March-2023 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n

The written Program of Veterinary Care (PVC) for dogs is missing required elements. The PVC does not include an appropriate schedule for the sampling and treatment of intestinal parasites. The current iteration of the PVC in use at the facility states stool samples will be tested on 20% of the kennel or as needed. The schedule does not prescribe how frequently stool samples should be obtained from the dogs. The facility has not performed routine fecal sampling since 2023 per available medical records. Failure to have an appropriate schedule for sampling of intestinal parasites in the PVC does not ensure that the facility is protecting the dogs from parasites and may allow illness to go undetected and untreated, which may cause pain and unnecessary discomfort to the dogs. The PVC must include a schedule for the treatment and sampling of parasites including worms, coccidia, and giardia approved by the attending veterinarian. Correct by 30 September 2025 This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspection(s) or by 15 November 2025 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n
