2.75(a)(1) (Non-Critical) – Records: Dealers and exhibitors.
The facility’s acquisition and disposition forms are incomplete. Records indicate that a litter of kittens was sold in August 2024 but there is no record of an adult cat on the Record of Acquisition of Dogs and Cats on Hand (APHIS form 7005). Accurate acquisition information is necessary for APHIS officials to make positive identification of animals during inspection and to properly trace their movements. The licensee shall make, keep, and maintain an approved method of documentation which fully and correctly disclose all information concerning each dog or cat purchased or otherwise acquired, owned, held, or otherwise in his or her possession or under his or her control, or which is transported, euthanized, sold, or otherwise disposed of by that dealer. The records shall include any offspring born of any animal while in his or her possession or under his or her control. The licensee must record all required acquisition and disposition information and have it accurately documented at all times. Correct by 11-22-24 This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by 20 Jan 2025 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n
