3.13(b)(3) (Non-Critical) – Veterinary care for dogs.
The applicant does not maintain complete medical records for all adult dogs. Medical records indicate that 16 adult dogs were overdue for the distemper and parvovirus vaccines. The applicant states that the animals had actually received these vaccines this year and were up to date but they did not record that information in the medical record. Failure to maintain adequate medical records may interfere with the ability of APHIS officials to determine if the animals are receiving adequate veterinary care. Complete medical records must be maintained and include the names of all vaccines and treatments administered and the dates of administration. Correct by documenting all treatments given and ensure that medical records are complete and contain the required information.
3.13(b)(4) (Non-Critical) – Veterinary care for dogs.
The applicant was unable to provide documentation of heartworm tests performed. Medical records indicate the Attending Veterinarian took samples for the heartworm tests on June 27, 2023. However, the applicant could not provide the results of the heartworm tests. Failure to document testing results may interfere with the ability of APHIS officials to determine if the animals are receiving adequate veterinary care. Medical records must be maintained and include the dates and findings/results of all screening, routine, or other required or recommended test or examination. Correct by documenting the results of all testing and ensure that medical records are complete and contain the required information. This is a 1st Re-License Inspection for a Class “A” license. The inspection and exit briefing were conducted with the applicant. All items must be in compliance within 2 more inspections or by August 5, 2024 or the applicant will forfeit the license fee and must wait 6 months to reapply. Please contact your inspector to schedule your next Re-License Inspection. Regulated activity can continue under your existing USDA license until it expires. n
